Submission Form

Final deadline for submissions is August 1st, 2012.

• Submission Fees — Submission fees are $325 per submission. Any late submissions will include a $75 late fee per entry. Late submissions include any submissions received between September 1st and September 15th, 2011. Payments are accepted by check or credit card only. If paying online with PayPal, submission company must be referenced to ensure proper payment credit. NO REFUNDS ISSUED FOR ANY REASON! *Please make certain to indicate the company name on the check of who is being submitted.*
• Submissions for company product/service MUST be LAUNCHED between August 1, 2011 and July 31st, 2012.
• All applications and payments MUST be received and paid in full to qualify. Any partial submissions forms or submissions lacking payment will be disqualified!
